1. All forms are available below in both Word and PDF format.
However, the following forms do not need to be completed using a Word or PDF format because they should be completed online by answering the questions on the CourtSA portal, which will automatically generate the form in the correct format:
a. Form 1 Information Lower Courts
b. Form 2 Information and Summons Lower Courts
c. Form 6 Information Higher Courts ex officio
2.Forms that include a lower case “e” or do not include any lowercase letter indicate that the form is in electronic format. These forms are intended to be completed electronically by downloading the form in Word format, completing the form on a computer and then lodging the form by uploading on the Court SA portal. Alternatively, for a limited introductory period, these completed forms may be printed and filed in hardcopy via the Registry.
These forms may be amended where applicable. Because the forms are in Word format, there is no technological limit on the ability to amend the form to make it apposite. For example, where a form includes options in square boxes, this indicates that you can keep any one or more options that are applicable and delete the remaining options that are inapplicable.
3. Forms that include a lower case “h” indicate the form is in hard copy format. These forms are intended to be printed and completed in hard copy. These forms will provide prompts to the party to “circle the applicable” option and include check boxes in which the form will ordinarily prompt the party to “mark appropriate selection with ‘x’”. Once completed, hardcopy forms may be scanned and filed electronically via the CourtSA portal or, for a limited introductory period, may be filed in hardcopy via the Registry.
